ATI Rage Pro 128 will not work under OS 9 or 10.4

I just bought a Rage Pro 128 card 630-2906 to replace one with a failing fan. The fan on the new card spins up but there does not seem to be any signal to the monitor. I tried it under 9.2.2 and also under 10.4.11 (separate partitions for each). The other numbers on the card are: TM9450DE5GHQA. Then on another sticker is: (01)727419630010(21)01945 07790. P/N 1026300101 013593. The ROM has a sticker with R128P 63001 (that is identical to the older card0.

Any advice?

Seems like a dead card, check any capacitors for signs of leakage.
